Author Description

张玉珍，广州中医药大学教授、博士生导9币，全国著名中医妇科学家罗元恺教授的学术继承人，享受国务院政府特殊津贴，是国家重点学科广州中医药大学妇科学术带头人之一，第五批全国老中医药专家学术经验继承工作指导老师。从事中医妇科医、教、研40余年，擅长中医药调经、防治反复自然流产、不孕症、卵巢早衰等。发表论文40多篇，出版专著22部，其中主编6部，副主编3部。主编普通高等教育“十五”“十一五”“十二五”国家级规划教材《中医妇科学》。主持研发了罗元恺教授经验方“滋肾育胎丸”与“田七痛经胶囊”。培养硕、博士研究生30名。

史云，医学博士，北京中医药大学东直门医院妇科主任中医师，硕士生导师。毕业于广州中医药大学，为岭南罗氏妇科流派第四代主要传承人之一，师从国家名老中医张玉珍教授。广东省中西医结合专业委员会生殖学会委员，世界中医药学会联合会生殖医学专业委员会理事。从事妇科临床、教学、科研工作10余年，具有深厚的中医妇科理论和丰富的临床经验，对月经不调、不孕症、流产、盆腔炎、卵巢早衰以及各类妇科疑难杂症具有丰富的治疗经验。主持国家课题1项，省部级课题3项，发表论文20余篇，参编专著3部。

《卵巢早衰的中医药防治》：
（一）寒邪
寒为冬令主气。凡在气温较低的季节，或由于气温骤降，人体不注意防寒保暖，则常易感受寒邪。冒雨涉水或汗出当风，过食寒凉生冷亦可使人受寒。寒为阴邪，易伤阳气；寒性收引，主凝滞，易使气血阻滞不通。寒邪致病，有外寒、内寒之分。外寒是指寒邪由外及里，伤于肌表、经络、血脉，或经期、产后血室正开，寒邪由下阴上客，入侵冲任、子宫，进而发生经行发热、经行身痛、痛经、月经后期、月经过少、闭经、产后身痛、不孕症等病证。内寒，是机体阳气虚衰，命火不足，温煦气化功能减退，或阴寒之气弥漫的一种病理状态。内寒的产生，与肾脾阳虚关系最大，一是由于阳虚失于温煦，导致各种虚寒之象及血脉收缩，血流减慢。二是由于气化功能减退，阳不化阴，代谢障碍，阴寒性病理产物如水湿、痰饮聚积。阳气的温煦和气化功能减退常导致闭经、月经后期、痛经、带下病、子肿、不孕症。
内寒与外寒既有区别又有联系。外寒以寒象为重，内寒以虚象为主。但外寒日久不散，阳气受伤，可致阳虚；素体阳虚内寒之人，又同气相求，易感寒邪，发为外寒证。
（二）热邪
热为阳邪，其性炎上，故热邪伤人，临床多以高热、出血、扰乱神明等上部症状为多；热邪易耗气伤津，正气损伤，津液亏乏，出现机能减退；热邪易生风动血，所谓“热极生风”，表现发痉、抽搐；热迫血行，出现血证。热邪致病也有外热、内热之异。外热为外感火热之邪，尤其以月经期、孕期、产褥期，热邪易乘虚而入，损伤冲任，发生经行发热、经行头痛、月经先期、月经过多、崩漏、带下、阴痒、子烦、子淋、产后发热。热邪结聚冲任、胞中，使气血壅滞，“热盛则肿”，“热盛肉腐”，则发为产褥热、盆腔炎或盆腔脓肿、阴疮、孕痈等病证。内热又称“火热内生”，主要是阳盛有余，机能亢奋，或五志过极化火，或六淫郁而化热，或瘀血化热，气郁化热以及阴虚生内热。火热内生，热伤冲任，迫血妄行，发为月经先期、月经过多、经行吐衄、经行头痛、经行情志异常、恶阻、胎漏、子烦、子痫、产后发热、阴疮等。
外热、内热显著不同。外热从外而人，热象较剧；内热从内而生，热象较轻。二者又互相影响，如内热之人，易感热邪。热邪致病，大多较急。其发生、发展和转归，取决于热邪的盛衰和人体正气强弱。

近20年来，卵巢早衰发病率有上升和年轻化趋势，属妇科临床比较疑难的病症。西医治疗本病常用激素替代，副作用较大，故求助于中医治疗的患者较多，而中医治疗亦不十分理想。本病严重影响年轻患者的身心健康和家庭和睦，所以研究这一疾病具有重要的社会意义。
张玉珍教授是当代中医妇科泰斗罗元恺教授的学术继承人，她从年轻时代就智慧超群，勤奋学习，博采众长，在继承和发扬罗老学术思想方面颇有心得，这就是她带领弟子们勇于攻克这一疑难病症的基础所在。
本书分上、中、下三篇，衷中参西，系统地介绍了卵巢早衰的中医药防治，并做了相关的专题研究和临床医案举隅，借以印证中医药防治卵巢早衰的理论和临床疗效。特别是介绍了中医药防治卵巢早衰的思路与方法，并从多渠道来防治这一疑难病症，体现了临床实用性。
本书的成功在于它是目前首部中医药防治卵巢早衰的专著，它体现出有继承、有创新、有理论、有实践的特点。本书文字不多，但分量厚重，贯穿着三代中医人对本病理、法、方、药的探索；在编书的形式上别具一格，值得借鉴。
此外，本书给我的启迪是：若经过进一步的梳理，可编入教材或教参，但建议首先要确定一个中医病名，与西医称的卵巢早衰相对应，以便构成中医学疾病系统，如称之为“天癸早竭”，特提供学术界商榷。
