Book Abstract

磨牙后区内可能有一下颌血管分支经过，如远中切口延及升支前缘且较偏舌侧时，可导致术中出血多而影响术野，应予以避开。
颞肌肌腱附丽大多止于磨牙后区的后部，亦有可达智牙远中，剥离难。翼内肌的前缘距下颌第三磨牙牙槽窝近。拔牙和冠周感染激惹颞肌肌腱和翼内肌，是造成开口受限的主要原因。
下颌第j磨牙通常牙冠颊面有2个发育沟，近中发育沟距根分歧近，并为牙冠的薄弱部位，常作为劈开牙冠时放置凿子的部位。
（二）影像学检查的指导意义
x线片应作为下颌第三磨牙拔除术必备的辅助检查资料。通过x线片可以判定牙阻生状况，包括倾斜状况、牙冠大小、牙冠与第二磨牙的关系、骨埋伏的程度、牙根数目和形态、周围骨质的密度。x线片可显示下牙槽管与牙根的关系和距离。同时要关注邻牙情况，主要是第二磨牙远中骨吸收、有无龋坏。还要注意第三磨牙周围是否存在其他病变。
根尖片作为口腔科的必备检查手段获取方便，可以较好反映第三磨牙的相关阻生信息。存在的不足主要是：投照的范围小，当第三磨牙冠周出现较大病变时不能反映全貌，而影响诊断。如出现低密度影像且根尖片上没有显示病变的完整边界，必须加拍其他投照位置以明确诊断，切不可贸然拔牙。由于投照角度造成的解剖影像的重叠，如根与下牙槽管、冠与邻牙、牙与颌骨，可能干扰医生对实际位置关系的判断，而导致拔牙设计的失策。根尖片中出现下牙槽管与牙根重叠时，易误认为根尖已突人管内，此时，应观察牙根的牙周膜和骨硬板是否连续，重叠部分的下牙槽管是否比牙根密度高、有无变窄等，以判断牙根是否已进入管内。
曲面体层片显示范围广，可做两侧对照，能较好提供拔牙所需信息。但在判定下牙槽管与牙根关系方面仍无法提供清晰的信息。而且其放大率也会带来一些判断上的误差。
锥形束cT可以准确判断牙根与下牙槽管的三维位置关系和准确距离。对于诊断下颌第1磨牙引起的第二磨牙根吸收有不可替代的作用。
（三）手术的常规步骤
下颌第三磨牙拔除术术前应仔细检查局部和全身情况，拍x线片。
依据所获得的临床信息进行阻力分析与拔牙设计。根据临床检查和x线检查，对阻生下颌第三磨牙进行拔除时阻力产生的来源和部位加以分析，并预先设想解除这些阻力的方式和途径是顺利完成阻生智牙拔除的必要步骤和基础。制定手术方案的意义在于使手术的每个环节和步骤都具有针对性，提高手术的效率，尽力减小创伤，预防并发症，最终保障手术成功。
一般拔除下颌阻生第三磨牙的阻力产生的部位有冠部阻力、根部阻力和邻牙阻力。阻力的来源有软组织阻力和骨组织阻力。
通过临床检查可以预估冠部软组织阻力的大小。冠部的软组织阻力主要是妨碍牙向骀向脱位。覆盖于冠部的软组织有一定的让性，有时比较松软，这种情况只要做好软组织的分离就可拔出智牙。但大多情况下龈片比较坚韧，当软组织覆盖牙冠远中超过1／2时，一般要考虑切开龈片解除阻力。
冠部的骨阻力主要在冠的远中和颊侧。远中去骨最少要超过牙颈部，颊侧去骨一般要暴露牙冠的近中颊沟。
