Book Abstract

【诊断标准】
未经诊断或未经控制的糖尿病患者，常有下面的口腔表现。
（1）口腔干燥，多伴有味觉障碍，可有颊黏膜痛及胀感。
（2）腮腺可肥大，唾液分泌的量和成分也可改变，如唾液和龈沟液的含糖量也会随血糖升高而增高。
（3）白色念珠菌病为糖尿病的常见并发症。
（4）糖尿病患者，在局部刺激因素相似的情况下，牙周病发生率及严重程度均大于非糖尿病者。
（5）血糖控制不佳和有严重全身合并症的糖尿病患者，其牙周炎患病率更高，病变程度更重。附着丧失更多、更快，也易发生牙龈炎症，如牙周溢脓，急性或慢性、单发或多发的牙周脓肿等。
【治疗原则】
重度牙周炎对糖尿病的血糖控制不利，也可能增加糖尿病患者伴发心血管疾病的危险；另外牙周炎治疗有助于糖尿病患者血糖的控制，特别是重度牙周炎患者而血糖又控制不佳者，更应积极进行牙周炎治疗。
（1）未被诊断或控制不佳的糖尿病患者，了解全身症状和体征，必要时与内科医生共同诊治。对于已明确的糖尿病患者，了解患者血糖的控制水平、用药和治疗史。
（2）建议患者按医嘱服药，控制血糖。糖尿病控制不佳者，在牙周治疗时，考虑辅以全身使用抗生素。
（3）糖尿病患者可进行常规的牙周治疗，已控制的糖尿病患者，一般对牙周治疗反应良好，能保持牙周健康。
（4）尽量减少精神紧张和焦虑。
（5）对患者进行有关牙周炎与糖尿病相关的健康教育，让患者认识到患牙周炎可影响血糖的控制，同时进行牙周炎治疗和健康促进，以重建和保持牙周健康。

本书的第一版系于2002年3月出版发行，距今已过10个春秋。近年来，伴随着我国经济、社会的全面进步，科学技术创新的潮流已呈现趋势化，口腔科各专业的发展也取得了长足的进步。为适应医学科学的发展，适应医疗工作的开展，适应临床医师的培训及执业医师定期考核，我们对全书内容进行了修订和补充。
本书的修订以强调“三基（基本理论、基本知识、基本技能）”为宗旨，以明确定位和体现整体优化为原则，着重突出以下特点：①内容精炼概括，力求全面覆盖；②理论联系实践，循序渐进指导；③专业衔接紧凑，避免重复脱节；④体现学科发展，力争知识更新。
按照口腔科专业的设置，本书共分32章，其中新增儿童口腔病、口腔预防、口腔颌面医学影像诊断和口腔病理诊断共4个章节，删除了目前临床操作中已很少使用的牙髓塑化疗法和干髓术的内容。为进一步体现学科发展方向，口腔黏膜病新增了性传播疾病口腔表征和系统疾病口腔表征。与第一版相比，本书删减和新增知识点共百余处，此处不再进行赘述。
